SACKETS HARBOR CENTRAL SCHOOL HEALTH SERVICES

Dear Parents,

New York State law requires that students who are:

  • new entrants to the school district
  • at the following grade levels: K, 1, 3, 5, 7, 9 and 11th grades
  • in need of working permits
  • participating in 7-12th grade interscholastic sports
  • are referred to the Committee for Special Education and for triennial reviews

Receive a physical examination during that school year. Students who do not have proof of a physical within the first thirty days of the school year will be screened by the school nurse and referred to the school physician for the required physical.

For your convenience, you may download the Mandated Health Physical Form if you choose a private physical.  Please note that the physical form also requests immunization information from your health care provider to complete. Parental permission for authorization for use or disclosure of protected health information (HIPPA) may be included with the physical form, if it is not on file for the current school year.  Please return any completed doctor/dental forms to the health office.  If the forms are left at the doctor’s/dentist office, please have a stamped envelope with the school’s address so the physical/dental exam may be sent to the school nurse.

All physicals and dental forms should be dated and have the doctor’s printed name/office stamp at the bottom.

If your child has a physical scheduled during the school year, please notify the Health Office with the date of the scheduled physical exam.  If you have any questions, please call the Health Office at 315-646-3575. Your physician may also fax the physical to 315-646-1038.
